A case management service has been introduced into Veterans' Affairs New Zealand to address the concerns regarding the wider health needs of veterans and their families.
Case management is designed to serve veterans who, as a result of their service, have either complex psychological and medical issues, or who are frail and elderly and need support to manage in their own homes. Veterans do not need to be in receipt of a War Disablement Pension to access this service.
In addition, case management facilitates access to counselling for the families of veterans who have been affected by their parent/partner's Service.
Children with spina bifida and, or cleft lip/palate, who have a parent who served in Vietnam or Operation Grapple can access additional assistance including genetic counselling.
Dedicated case managers are assigned to assist veterans with their holistic health needs. The case managers are qualified and experienced professionals who have an understanding of the issues veterans are facing. Case managers will design an individual care plan to help the veteran access the health and social services required to ensure personal independence and best quality of life.
