Transition Health Check

A post-release health assessment for veterans.

The transition health check is available to veterans who have recently left service within the last three years.

It aims to encourage them to sign up with a GP’s practice, as many will not have a GP. This is because while they are in the NZ Army, Navy or Air Force, all their health care is provided by the NZDF. 

Who can get this

A veteran will be eligible if they:

  • have Qualifying Operational Service, and
  • have left the NZDF (any of the services) within the last three years.

Qualifying Service explained

Veterans can prove their eligibility by displaying their Veteran Card

Eligible veterans will receive an email containing a digital Veteran Card, which they may print off. This is a personalised card that contains their name and service number. We automatically send the Veteran Card to members of the NZDF who are released from service and transitioning into civilian life.

What they can get

We will cover the cost of three annual GP health assessments (one per year over a three-year period).

How to process and invoice the patient

  • Step 1 The veteran makes an appointment and shows you their Veteran Card

    The veteran will have received an email that asks them to book an appointment with a GP. The email contains a digital Veteran Card which confirms their eligibility.

  • Step 2 Provision of the health assessment

  • Step 3 Invoice us

    Green Cross Health processes invoices on our behalf for services provided by doctors within New Zealand.

    You need to send your invoice details to Green Cross in a specific format. This helps ensure you get paid correctly and on time.

    If you haven’t sent an invoice to Green Cross Health before, you should get in touch with them to discuss their requirements.

    Contact Green Cross:

    On our behalf, Green Cross Health will pay you on the 20th of the month after they have received your invoice.

  • Step 4 Book the veteran for two further health assessments

    Make appointments with the veteran for next year and the following year. This ensures that they receive the three annual health assessments that they are entitled to.
