This form can be completed by a veteran themself or by someone on their behalf. Throughout the form, 'you' refers to the veteran that is being registered.
Before you start
- You are not immediately registered when you submit this form. We need to confirm your Qualifying Service before we accept your registration.
- If your registration is successful, it does not mean that your future claims for support will be accepted. All claims are assessed on a case-by-case basis.
- If you require urgent help now, there are resources you can access without waiting for confirmation.

You don't need to provide personal identification like a passport or driver license.
We'll contact you to confirm whether your registration was successful or not. We may ask for more information.
